A cornea transplant (keratoplasty) is a surgical treatment that replaces a portion of your cornea with donor corneal tissue. The clear, dome-shaped surface of your eye is known as the cornea. It is a key aspect of your eye's ability to see well because it is where light enters your eye.
Endothelial Keratoplasty with Descemet Stripping (DSAEK) DSAEK is a partial thickness cornea transplant surgery that involves removing the patient's Descemet membrane and endothelium, then transplanting donor corneal endothelium and stroma.
Disease Overview:
Damaged endothelial layer
The corneal endothelium's main physiological purpose is to enable solute and nutrient leakage from the aqueous humour to the cornea's more superficial layers while also pumping water in the opposite way, from the stroma to the aqueous humour.
Corneal endothelium damage results in persistent corneal edoema and bullous keratopathy. Chemical damage to the eye surface can cause severe inflammation, epithelium necrosis, and a lack of limbal stem cells.
Disease Signs and Symptoms:
- Vision is hazy.
- Eye irritation, stinging, or burning is a common symptom.
- Do you get the sensation that something is in your eye? (may be caused by a scratch or something in your eye)
- Sensitivity to light.
- The appearance of redness in the eyes.
- Eyelids that are swollen.
- Increased weeping or watery eyes.
Disease Causes:
Penetrating or blunt trauma, congenital hereditary endothelial dystrophy (CHED), iridocorneal endothelial (ICE) syndrome, refractory glaucoma, past unsuccessful corneal grafts, and herpes simplex virus endotheliitis are some of the conditions that can cause endotheliitis. FECD is the most prevalent main cause of corneal endothelial dysfunction. PBK is the most prevalent secondary cause of corneal edoema, which reflects the rise in popularity of cataract surgery and intraocular lens implantation over the last two to three decades. Glaucoma and its treatment (medical and surgical) have a negative impact on the corneal endothelium, which can limit corneal transplant life.
Disease Diagnosis:
A number of equipment, including as contact or non-contact specular microscopes, confocal microscopes, or slit lamp biomicroscopes, can be used to study the corneal endothelium via specular reflection.
Disease Treatment:
Although medicinal therapy can help reduce symptoms, corneal endothelial dysfunction can only be treated through corneal transplantation, which can be done as full-thickness penetrating keratoplasty (PK) or selective keratoplasty.
