Decompression Spine Anterior Approach Dorsal:
The anterior thoracic spine, vertebral bodies, intervertebral discs, spinal canal, and nerve roots are all easily visible and accessible with an anterior approach. This method is now employed in the surgical treatment of thoracic disc disease, vertebral osteomyelitis or discitis, fractures, and tumours of the vertebral bodies, allowing for adequate neural decompression and spine stability.
The anterior approach to scoliosis surgery refers to the surgeon approaching the spinal column from the front rather than the rear.
To reach the compressed nerve, the surgeon makes an incision (cut) down the afflicted area of spine to the lamina (bony arch of your vertebra). A portion of the bone or ligament pushing on the nerve will be removed, and the nerve will be pushed back towards the centre of the spinal column.

Pott's disease
Pott's Illness, also known as tuberculosis spondylitis, is a rare infectious disease of the spine caused by an infection outside of the spine. Pott's Disease is a condition that includes several vertebrae and is caused by a combination of osteomyelitis and arthritis. The anterior part of the vertebral body close to the subchondral plate is the most common location of involvement in the lower thoracic vertebrae. Vertebral collapse is a potential side effect of this condition, and when it happens anteriorly, anterior wedging develops, resulting in kyphotic deformity of the spine. Compression fractures, spine abnormalities, and neurological insults, including paraplegia, are all potential side effects.
Disease Signs and Symptoms:
The thoracic, or higher, spine is commonly affected by Pott's Disease. Fever, nocturnal sweats, and weight loss are early indications of TB. The most common sign that TB has spread to the spine is severe back discomfort.
Patients will have trouble standing as the condition progresses. They may walk with a rigid, unyielding posture and may have edoema at the infection site, which is often accompanied by numbness or paralysis in the legs. Pott's Disease can cause a significant curvature of the spine and limb paralysis if left untreated.

Cervical spine tuberculosis is a less frequent kind of tuberculosis, occurring in around 10% of cases, but it can be more devastating since severe neurological consequences are more prevalent. Disease Causes
Inhaling air with Mycobacterium tuberculosis (Mtb), the organism that causes tuberculosis, causes tuberculosis to start in the lungs. If the infection progresses from the lungs to the spine, it can lead to Pott's Disease.
Tuberculosis (TB) transmission: Mtb infects the spine's joints, resulting in a kind of spinal arthritis. When the disc of cartilage between two adjacent joints becomes contaminated, it is unable to get the nutrients it need to survive. The disc degenerates and compresses, causing vertebral constriction, ultimate vertebral collapse, and spinal cord injury. Spinal tuberculosis, if left untreated, can cause severe abnormalities, nerve damage, and possibly paralysis.
Disease Diagnosis:
Injection of a pure protein derivative in the Mantoux Test (Tuberculin Skin Test) (PPD). Patients with Pott's illness who are not infected with HIV had positive results in 84-95 percent of cases.

Microbiology Research
To confirm a diagnosis, microbiology tests are done. Acid-fast bacilli (AFB) are stained in bone tissue or abscess samples, and organisms are isolated for culture and susceptibility testing. Percutaneous sampling of damaged bone or soft tissue structures can be directed by CT-guided procedures; nevertheless, these research findings are favourable in only about half of the instances.

Although posterior fusion has been effective in treating kyphosis, several fusion failures have occurred due to preoperative infection and excessive degrees of kyphosis. Hodgson and Stock's anterior method has also shown to be quite effective.
Depending on whatever part of the spine is damaged, different surgical procedures are used. A transoral or extreme lateral approach is used in the upper cervical spine, which usually necessitates concomitant occipito-cervical fusion to avoid collapse, instability, and delayed deformity. Standard anterior cervical methods are frequently used to treat midcervical lesions, with satisfactory outcomes. In patients with involvement of the lower cervical/upper thoracic spine, transsternal, transmanubrial, or lateral extracavitary techniques are used.
Transthoracic, extraplural anterolateral, and extended posterolateral techniques are used by thoracic spine surgeons. Due to the nature of the spinal deformity and ease of access to the spine, the posterolateral technique is more commonly used in severe instances of kyphosis.
