Alexandra Hospital, Singapore | Singapore , Singapore

Profile (Overview)

Since 1 June 2018, Alexandra Hospital (AH) has been a part of the National University Health System (NUHS). It will be a 326-bed facility that will offer seamless, centralised care to about 100,000 residents of Singapore's southwest, Queenstown precinct (the country's oldest housing estate), and other parts of the island. As Singapore's first integrated general hospital, Alexandra Hospital offers comprehensive, seamless care from acute, sub-acute, through rehabilitative settings, minimising the need for patients to move to other medical facilities. Our clinical model is program-based and team-led. This implies that any patient who enters Alexandra Hospital will be assigned to at least one of our five main clinical programmes, and will receive care from the same physician and care team as part of that programme. These are our five clinical programmes: Be better (preventive care), Get better (acute care), Live better (chronic care), Age better (healthy ageing), and Cope better are all positive life improvements (Supportive and Palliative Care) The hospital's care managers work with patients, families, and community partners through the Community Care Coordination Unit (C3U) to plan and offer ongoing medical and social support to individuals who require it, so enabling them to maintain their health at home and in the community.
